Grants will only be given to Causes (defined below) in the 50 U.S. States and the District of Columbia. Any grants submitted for Causes with locations outside of these areas will not be reviewed.
Anyone over 13 years of age within the 50 U.S. States or District of Columbia is eligible to submit a Cause, including State Farm® agents, employees and their immediate families.
Each entry must fall under one of three broad buckets: Education, Safety, or Community Development.
All entries will be read and reviewed by the State Farm Review Committee. The top 200 entries as chosen by the State Farm Review Committee will be featured on www.neighborhoodassist.com (the "Neighborhood Assist Website") on Wednesday, August 16 for public voting.
The top 40 vote-getting Causes will receive $25,000 each in grant money to complete their project.
Your community's participation will be key in order for your Cause to win!
The 2017 "State Farm Neighborhood Assist" Program (the "Program") begins at 12:00:01 A.M. ET on Wednesday, June 7, 2017 and ends at 11:59:59 P.M. ET on June 21, 2017 or when 2,000 entries have been submitted, whichever comes first.
Contest submissions may be made by legal residents of the 50 U.S. States and the District of Columbia who are 13 years old or older as of June 7, 2017 (each an "Submitter") with internet access and a valid email address at the time of entry. ENTRIES ARE MADE ON BEHALF OF THE COMMUNITY (City, State) OR 501(C)(3) CHARITY (THE "CAUSE") DESIGNATED BY ENTRANT AT THE TIME OF ENTRY AND SUBMITTER IS NOT ELIGIBLE TO RECEIVE GRANT MONEY. IF NO 501(c)(3) REGISTERED ORGANIZATION IS SELECTED, ONE WITHIN ENTRANT'S GEOGRAPHICAL AREA WILL BE ASSIGNED. Void where prohibited. Program is subject to all federal, state and local laws and regulations. The State Farm Review Committee will not be eligible to submit a Cause or vote for a Cause.
Nonprofit Administrator Eligibility.
Program Sponsors follow the Good Neighbor Citizenship® Company Grants funding guidelines. All 501(c)(3) registered organizations designated as eligible for State Farm funding are eligible to be considered as possible administrators of a Neighborhood Assist grant. How to Enter.
To enter, visit the Neighborhood Assist Website at www.neighborhoodassist.com to (i) complete the application form ("Application"), (ii)designate a Location of Cause (city and state) to receive a grant (if one is won), (iii) indicate acceptance of the Program rules, and (iv) upload up to three (3) photos related to your Cause (minimum of one (1) photo) (collectively the "Entry"). By submitting an Entry, you agree to abide by the Program's Official Rules. Submitter may suggest a nonprofit 501(c)(3) organization to act as administrator of the grant. Your suggested nonprofit organization will not be judged as part of the Application. All submissions must be in English. ONLY THE DESIGNATED COMMUNITY OR NONPROFIT ORGANIZATION, AND NOT THE SUBMITTER, IS ELIGIBLE TO WIN A GRANT IN THE PROGRAM. BY SUBMITTING AN ENTRY, SUBMITTER ACKNOWLEDGES THAT HIS/HER ENTRY MAY BE POSTED ON THE NEIGHBORHOOD ASSIST WEBSITE AT WWW.NEIGHBORHOODASSIST.COM OR ANY OTHER WEBSITE(S) OR PAGE(S) AUTHORIZED BY THE SPONSOR FOR THE PROGRAM (COLLECTIVELY "THE WEBSITE"), AT SPONSOR'S DISCRETION AND CAN BE VIEWED BY OTHER WEBSITE USERS. Submission of an Entry grants Sponsor, its subsidiaries and affiliates and their respective agents, and anyone designated by Sponsor, an unlimited, worldwide, perpetual license and right to publish, use, modify, edit, adapt, copy, exploit, and publicly display the Entry in any way, in any and all media, without limitation, and without consideration or acknowledgement to the Submitter. Sponsor reserves the right to modify or edit any Entry, in whole or in part, for content, clarity, or any other reason, and each Submitter waives any and all "moral rights" in and to his or her Entry. Only Entries submitted on the Neighborhood Assist Website will be accepted. No mailed-in, e-mailed, facsimile or other entry methods will be accepted.
You may submit one entry into each of the three Program categories (Education, Safety, and Community Development.) Each entry must be for a different Cause and be submitted separately and be in accordance with the terms and conditions stated herein. The "first" (as determined by the time stamp for submission) entry into each category by one submitter will be reviewed; all subsequent submissions by that same name and email address in that same category will be disqualified.
Entries must be received either by 11:59:59 P.M. ET on June 21, 2017, or before the submission cap of 2,000 submissions is reached (whichever occurs first) to be eligible.
By submitting an Entry in the Program, you hereby warrant and represent that your Entry conforms to the entry requirements set forth in these Official Rules. Entries must be your original and sole creation, and all right, title and interest including but not limited to the copyright and moral rights in any Entry must be owned solely by you. You are responsible for obtaining written permission from any person appearing in any submitted photo to use their likeness in a manner consistent with the rights granted in these Official Rules, and providing such written consent to Sponsor upon request. Entries may not include any: (a) copyrighted works without permission of the copyright owner, (b) false, libelous or defamatory statements about any person or any third party or depict any person, product or entity in a false light; (c) third party trademarks or logos that suggest affiliation with any trademark owner without permission of such owner or that dilute the value of any trademark; (d) profanity, nudity, depictions of violence, threatening language, inappropriate, indecent, harmful, discriminatory, or obscene content as determined by Sponsor and Administrators, in their sole and absolute discretion; (e) communicate messages or images inconsistent with the positive images and/or goodwill with which Sponsor wishes to associate; (f) depiction of an act which is dangerous or suggests any person to undertake a dangerous activity; (g) must not infringe upon any third party's rights, including but not limited to, copyright or rights of privacy or publicity; and (h) cannot defame, misrepresent or contain disparaging remarks about Sponsor or its products, or other people, products or companies. All Entries must be in keeping with Sponsor's image, as determined by Sponsor in its sole and absolute discretion. Sponsor shall have the right, but no obligation, to review the Entries for compliance with these entry requirements. At the judges' discretion, entries including unsuitable content or that are in, or alleged to be in, violation of third party rights (including, but not limited to, copyrights) will be disqualified. Sponsor and Administrators retain sole discretion as to what constitutes unsuitable content and will not be liable in any way for disqualified entries.
Sponsor is not responsible for lost, stolen, illegible, unreadable, misdirected or late Entry submissions. With respect to the Website or Entry, Sponsor assumes no responsibility for computer system, phone line, hardware, software or program malfunctions or other errors, failures of any kinds, delayed computer transmissions or network connections that are human or technical in nature. ENTRIES POSTED TO THE WEBSITE ARE THE VIEWS/OPINIONS OF THE INDIVIDUAL SUBMITTER AND DO NOT REFLECT THE VIEWS OF SPONSOR IN ANY MANNER. By submitting an Entry, you agree that your disclosure is gratuitous, unsolicited and without restriction, and will not place Sponsor under any obligation, that Sponsor and others are free to disclose the ideas contained therein on a non-confidential basis to anyone or otherwise use the ideas without any additional compensation to you. You acknowledge that, by acceptance of your submission, Sponsor does not waive any rights to use similar or related ideas previously known to Sponsor, or developed by its employees, or obtained from sources other than you. Any waiver of any obligation here under by Sponsor does not constitute a general waiver of any obligation of Submitter. By entering, you acknowledge that Sponsor has no obligation to use or post any Entry you submit.
Identical or similar submissions coming from the same city and state/province will not be disqualified due to duplication. The State Farm Review Committee will retain full decision-making ability on ranking the top causes, so it is possible that nearly identical causes from the same area comprise the top 200.
Submission Uses and Intellectual Property Rights.
By entering, Submitter grants permission for the use of his/her Entry, in whole or in part, and his/her name, and the likeness of any person depicted in the Submission, for use by Sponsor and any third party designated by Sponsor in any and all media now known or hereafter devised without territorial or time limitation, including but not limited for posting on the Internet and the Neighborhood Assist Website and/or any website in connection with this Program, in the advertising, promotion, and publicity of the Program, and commercial uses, and otherwise as stated in these Official Rules (as solely determined by Sponsor). Each Submitter warrants and represents that he/she has all right and authority to grant such permission. Notwithstanding anything to the contrary, however, there is no obligation of the Sponsor or any other person or entity to post or make any use of any Entry.
Submitters and/or administrator organizations may withdraw their name and submission from consideration at any time by sending an email to firstname.lastname@example.org, requesting the withdrawal.
In the interest of fairness and accessibility and to align with the State Farm Grant Program, returning grantees cannot win more than two consecutive years in a row. All past recipients of either a State Farm Cause an Effect or State Farm Neighborhood Assist grant are eligible to apply again for the Program as long as they have not won in more than two consecutive years. This is to be judged by the non-profit organization receiving the grant monies as opposed to any specific limit on the individual submitting the Cause. Once a non-profit organization has won two years in a row, they will not be eligible for the following year of the Program. For past recipients of either Cause an Effect or Neighborhood Assist Grants, the two-year limit is effective immediately, and will not be considered by the State Farm Review Committee for selection in the event that the sponsoring non-profit organization applies for the Program.
The State Farm Review Committee will review and score the submissions once the 2,000 submission cap has been met or the submission period has ended, whichever comes first. Based on publicly available criteria, the State Farm Review Committee will select their top 250 Causes, 200 of which will go onto the State Farm Neighborhood Assist Website for public voting. Sponsor will reduce the number from 250 to 200 using the same criteria. Each cause submitted will be read and reviewed by at least three members of the State Farm Review Committee, and the scores will be averaged together to obtain a total score.
The State Farm Review Committee will score grants based ONLY on the three questions below. No entry will be discriminated against based on the answering of any of the other questions in the application. The image submitted with the entry will not be made available to the State Farm Review Committee during their scoring. No entry will be discriminated against on the basis of ethnicity, age, location, or any other determinant factor than the answer from the questions below.
Scoring Rubric and Weighting.
The State Farm Review Committee will use the below rubric for scoring. Each of these questions will be scored with equal weighting on a 7-point scale, to determine a complete perfect score of 21.

State Farm Human Resources will, using the State Farm Review Committee rankings, match the "Causes" to an appropriate nonprofit partner in that local area. This process will be done from the top scoring submission down, so that the integrity of the State Farm Review Committee rankings remains intact. State Farm Human Resources will try to align with one of the suggested nonprofit local branches first. If there is no branch within a reasonable distance from the Cause location, State Farm Human Resources will then go to the custom nonprofit submitted by the user, if they submitted one. If they did not submit a nonprofit, State Farm Human Resources will use their nonprofit database to find a suitable nonprofit partner in that particular community.
Three State Farm Review Committee members will read each application. They will score each of these questions on a point scale of 1-7. They will rank all submissions they receive by average total score from the three readers, and each question will be weighted the same. The State Farm Review Committee will review all submissions based on 3 basic criteria, listed on the submission form itself. To eliminate ties, each question has been ranked in order of tie-breaking power. In the case of a complete tie, the State Farm Review Committee will compare scores for questions #1 (if those are exactly the same, then #3 and #2, in that order.)

Out of the top 200, each "Bucket" will have at least 15 Causes, determined by the State Farm Review Committee ranked scores. Should the total number of Cause submissions not reach 40, State Farm reserves the right to award less than 40 grants. If the total number of Cause submissions exceeds 40 but is less than 200, the State Farm Review Committee reserves the right to select at most all, and at least 40 Cause submissions to be placed on the Website for public voting.
All entrants will be notified of their submission status via email to the email address included in the submission, on or about Thursday, August 15, 2017.
Beginning Wednesday, August 16, 2017 at 12:00:01am ET, anyone within the 50 U.S. states and District of Columbia with a valid email address is eligible to vote for a Cause. This includes State Farm agents, employees and their immediate families. Each person can vote up to (10) times per day. These votes can be used all on one Cause, or spread out in any way among multiple Causes in multiple Buckets. Votes do not roll over. Users can come back each day to distribute their 10 votes. A "day" as defined by these program details is from 12:00:01 am until 11:59:59 pm ET each day. At the end of each day, any remaining unused votes are lost.
Sponsor is not responsible for ensuring the accuracy of the voting, and Submitters agree that Sponsor is not liable for any malfunctions, inaccuracies, or errors related to the voting or voting process or the results of the consumer voting or voting process.
The forty (40) Causes earning the highest number of votes by 11:59:59 P.M. ET, Friday, August 25, 2017 will be designated the winners of a "Neighborhood Assist" grant, at $25,000 each. There are no limits as to which of the three issue "categories" the winners fall within; winners are defined as the top 40 vote-getting causes across the entire initiative. In the event of a tie during the general public voting phase, tiebreaker will be based upon the raw score of the rubric completed by the State Farm Review Committee during the State Farm Review Committee phase.
Affidavit of Eligibility and Liability/Publicity Release.
All Cause submissions are subject to confirmation and verification of eligibility by Administrators and Sponsor. The top 200 Causes, and the corresponding nonprofit agencies matched with the "Cause" submission, will be required to provide confirmation (via e-mail) that they would adhere to all grant standards should they eventually make it into the top 40. If the nonprofit agency does not sign and return the release within seven (7) calendar days of receipt, or if the Sponsor is unable to find a non-profit agency that would administer a grant for the stated Cause, the grant will be forfeited and an alternate Cause chosen. By submitting an Entry, each Submitter agrees, and by accepting a grant, each nonprofit organization agrees, to allow and hereby grants to Sponsor, its affiliates and designees the right to use of his/her/its name and Entry (as applicable), in full or in part, in any media, now known or hereafter created, for any and all purposes, without compensation or acknowledgement, including, but not limited to promotional and advertising purposes, except where prohibited. If Submitter is under the age of 13, Sponsor will contact Submitter to obtain parental consent for publicity release in the same terms as listed above.
General Terms and Conditions.
All grants are non-transferable and will only be awarded in the name of a verified Cause organization. The Cause organizations are to be determined by a panel of State Farm community relations employees, in accordance with State Farm Charitable Giving guidelines. Submitted suggested organizations are not necessarily the organizations who will administer the grant on behalf of the community. Local branches of the national partner organizations suggested by State Farm on the Neighborhood Assist Website will be given preference for administering the grants. If a particular city does not have a local branch of a national partner organization within a reasonable distance, then the nonprofit suggested by the Cause submitter will be vetted as a possible administrator of the grant, and State Farm reserves the right to select an administrator of its choosing if no suitable branch can be found from those specific sources, or to disqualify the entry and select an alternate winner. If Sponsor is unable to find organizations to administer eligible grants, or if an insufficient number of eligible entries are received within the entry period, not all grants will be awarded. If any grant notice is returned as undeliverable, or if potential winning Cause organizations' information provided is unintelligible or incorrect, grant will be forfeited, and another Cause selected. By participating in the Program, Submitter agrees, and by accepting a grant, Cause organization agrees, to release, hold harmless and indemnify Sponsor, Administrator, and each of their respective parent companies, affiliates, subsidiaries, distributors, sales representatives, retailers, advertising, sales promotion and judging agencies and the employees, directors, officers and agents of each ("Sponsor and Administrators") from and against any and all liability whatsoever for any injuries, losses, harm, damage (including but not limited to property damage, personal injury and/or death), claims, actions, costs and expenses (including without limitation, all attorney's fees and court costs) arising directly or indirectly from or in connection with his/her participation in the Program, use of the Entry, acceptance, possession or use/misuse of the grant or participation in any grant-related activity. Winning Cause organizations are solely responsible for any local, state, provincial or federal taxes on grants (if any). Sponsor reserves the right to take such action as it deems necessary to enforce these Official Rules and ensure the fair operation of the Program, including without limitation, barring and disqualifying participants who do not comply with these Official Rules. By entering Submitters agree, and by accepting a grant, winners agree, that (1) all issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, participant's rights and obligations, or the rights and obligations of the Sponsor in connection with the Program, shall be (i) governed by, and construed in accordance with, the laws of State of Illinois without giving effect to any choice of law or conflict of law rules, (ii) resolved individually, without resort to any form of class action, and exclusively by the United States District Court for the Central District of Illinois or the appropriate Illinois state court; (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs, but in no event including attorneys' fees, disbursements or court costs; and (3) under no circumstances will Submitter or Cause organization be permitted to obtain awards for, and Submitter and Cause organization hereby waives all rights to claim, punitive, incidental and consequential damages, and any other damages, other than for actual out-of-pocket costs.
Additional Warranties and Indemnity. Anyone submitting Entries to this Program warrants and represents that publication of the Entry via various media including Web posting, will not infringe on any third party rights (e.g., music, trademarked/branded clothing). By entering this Program, each Submitter agrees to release, indemnify and hold harmless, Sponsor and Administrators from any claims related thereto. Sponsor has the right, but not the obligation, to remove any posts or to disqualify any entries it deems to violate this warranty.
Additional Terms and Disclaimers.
Sponsor reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Program or Website or to be acting in violation of the Official Rules. Sponsor reserves the right in its sole discretion to cancel or suspend this Program, in its entirety, if the Program cannot be operated as planned or should virus, bug, tampering, unauthorized intervention, fraud, technical failures, or other causes beyond Sponsor's control corrupt the administration, security, fairness, integrity or proper play of the Program. Sponsor and Administrators are not responsible for incorrect or inaccurate entry information whether caused by Internet users or by any of the equipment or programming associated with or utilized in the Program by any technical or human errors, which may occur in the processing of the entries in the Program. The Sponsor and Administrators are not responsible for any typographical or other errors in the offer or administration of the Program, including, but not limited to, errors in the printing or posting of the offer or Official Rules, the selection and announcement of any winner, or the distribution of any grant. SPONSOR AND ADMINISTRATORS DISCLAIM ANY LIABILITY FOR DAMAGE TO ANY COMPUTER SYSTEM RESULTING FROM ACCESS TO OR THE DOWNLOAD OF INFORMATION OR MATERIALS CONNECTED WITH THE PROGRAM. SPONSOR AND ADMINISTRATORS ARE NOT LIABLE FOR ANY INJURIES THAT MAY OCCUR FROM THE COMPLETION OF THE PROJECT ASSOCIATED WITH ANY WINNING CAUSE. EACH SUBMITTER AGREES TO EXECUTE ANY DOCUMENT CONTAINING THE TERMS AND CONDITIONS OF THESE OFFICIAL RULES.

The list will be live no earlier than Thursday, September 28, 2017 and will remain on the Neighborhood Assist Website for at least 30 days.
By clicking the e-mail communication box within the Neighborhood Assist Website, Submitter consents to receiving future e-mail communications regarding this or any other community-based programs or grant opportunities (in education, safety, or community development) from State Farm. Submitter will in no way be contacted regarding products or services from any information gathered during this Program.
